CHAS, Pioneer Generation (PG) and Merdeka Generation (MG) cardholders can receive subsidies for selected chronic conditions at participating General Practitioner (GP) clinics.
Chronic conditions are medical conditions that are generally progressive and require long-term care and medication. There are 23 chronic conditions covered under CHAS. They are:
- Allergic Rhinitis
- Anxiety+
- Asthma
- Benign Prostatic Hyperplasia
- Bipolar Disorder+
- Chronic Hepatitis B
- Chronic Obstructive Pulmonary Disease
- Dementia
- Diabetes Mellitus (DM) (including Pre-Diabetes)
- Epilepsy
- Gout
- Hypertension (High blood pressure)
- Ischaemic Heart Disease
- Lipid Disorders (e.g. High cholesterol)
- Major Depression+
- Nephritis/Nephrosis (Chronic Kidney Disease)
- Osteoarthritis
- Osteoporosis
- Parkinson's Disease
- Psoriasis
- Rheumatoid Arthritis
- Schizophrenia+

As chronic conditions are generally progressive and require long-term care and medication, it is important that patients manage them well through regular follow-ups and good lifestyle habits, to lower risks of complications, lessen medical bills, reduce stress and improve quality of life.
CHAS Benefits with Healthier SG
To further support patients with chronic conditions, enhanced subsidies are provided under Healthier SG.
Through the Healthier SG Chronic Tier Subsidy Framework, all CHAS, PG and MG cardholders can get common chronic medications at their enrolled Healthier SG GP Clinic, at prices comparable to those at polyclinics. The Healthier SG Chronic Tier will benefit cardholders who have higher medication needs and whose bills may exceed the current CHAS annual subsidy limits.
Only one subsidy tier may be used at each chronic visit. Residents may choose between the CHAS Chronic Tier or Healthier SG Chronic Tier, to apply the subsidy tier that best benefits them based on their medication needs. Pro-ration of the annual dollar subsidy balance will automatically apply upon switching between the two tiers.

Additionally, patients with conditions under the CDMP can use MediSave to fully pay for their treatment at their enrolled Healthier SG GP clinics, up to the MediSave500/700 limit. They no longer need to co-pay 15% of their bill in cash.
Certain GP clinics may also arrange for direct delivery of selected chronic medications under the Healthier SG Medication List to the enrollee's home. Delivery charges may apply.
